1. The undersigned application for membership to the International Association of Fire Fighters, Local F-121 hereby agrees to abide by the Local and International By-Laws and Constitution.
2. The undersigned application for membership to the International Association of Fire Fighters, Local F-121 hereby agrees to tender $30.59 as a payment of initiation fee.
   1. $14.84 1 Month per capita in advanced
   2. $15.00 Initiation Fee
   3. Please note that this also includes the credit card processing fee of 2.2% +.10 cents for every transaction
3. The undersigned application for membership to the International Association of Fire Fighters, Local F-121  hereby authorizes the Union to be my exclusive bargaining agent for collective bargaining for wages, hours and other conditions of employment.
4. The undersigned application for membership to the International Association of Fire Fighters, Local F-121 hereby agrees authorization of BI-WEEKLY payroll deductions for Union Dues
